Liver Function: The Central Metabolic Question

Ivermectin is processed almost entirely by the liver, specifically through the cytochrome P450 enzyme system — predominantly CYP3A4. This means that any pre-existing hepatic impairment has the potential to alter how the drug behaves in the body. In patients with compromised liver function, ivermectin may clear more slowly, leading to prolonged plasma concentrations and an elevated risk of adverse effects.

Patients with chronic kidney disease (CKD) may have altered drug distribution and protein binding, and the physiological complexity of renal impairment can interact with ivermectin's metabolic pathway in ways that are difficult to predict without baseline data.

For most otherwise healthy adults, renal function will not present a barrier to ivermectin therapy. But identifying a patient with previously undiagnosed CKD before prescribing — rather than after an adverse event — is precisely the kind of proactive clinical care that pre-treatment testing is designed to enable.

Parasitology Screening: Matching the Treatment to the Actual Condition

This is perhaps the most underappreciated element of pre-treatment evaluation, and it is where the clinical stakes are highest. Ivermectin treats specific parasitic infections — among them strongyloidiasis, onchocerciasis, lymphatic filariasis, and certain ectoparasitic conditions. The drug does not treat all parasites equally, and it is not effective against every organism that causes gastrointestinal or cutaneous symptoms.

More critically, there is one parasitic co-infection that transforms ivermectin from a routine treatment into a potential emergency: Loa loa, a filarial worm endemic to parts of Central and West Africa. Patients with high Loa loa microfilaremia who receive ivermectin can develop serious neurological complications, including encephalopathy. While this risk is most relevant to patients with travel or residence histories in endemic regions, it illustrates precisely why parasitological context matters.

For patients seeking ivermectin to treat suspected strongyloidiasis — a roundworm infection that is more prevalent in the United States than many clinicians realize, particularly among immigrants, veterans, and individuals with prolonged soil exposure — confirmatory testing via stool ova and parasite (O&P) examination or serology strengthens the clinical rationale for treatment and helps rule out co-infections that might complicate the course.

A prescriber who asks about travel history, geographic exposure, and prior parasitic infections is performing exactly the kind of individualized assessment that responsible ivermectin prescribing requires.

Complete Blood Count: A Broader Safety Net

A complete blood count (CBC) is not ivermectin-specific, but it provides valuable contextual data that informs safe prescribing. Eosinophilia — an elevated eosinophil count — is a common marker of parasitic infection and can help corroborate a clinical suspicion of strongyloidiasis or other helminthic disease. Conversely, abnormal white blood cell counts, platelet irregularities, or signs of anemia may flag underlying conditions that affect treatment planning.

Think of the CBC as the broadest lens in the pre-treatment evaluation. It rarely changes the prescribing decision on its own, but it adds important texture to the clinical picture.

How to Advocate for Proper Testing

Patients who want comprehensive pre-treatment evaluation should feel empowered to ask for it directly. A straightforward approach is to request, at the time of consultation, that the prescriber order a hepatic panel, a basic or comprehensive metabolic panel, a CBC with differential, and — where clinically relevant — stool O&P testing or parasitology serology.

If a prescriber declines or suggests these tests are unnecessary, that is an opportunity for a candid conversation. Ask specifically: Has my liver function been assessed? Is there any reason my kidney health might affect how this medication is processed? Given my travel or exposure history, is parasitology screening warranted?

Patients using telemedicine platforms to obtain ivermectin consultations — an increasingly common pathway — should be aware that some virtual providers may have limited visibility into prior lab results. Bringing recent bloodwork to a telehealth visit, or requesting that lab orders be issued as part of the consultation, closes that gap meaningfully.

The Bigger Picture

Ivermectin is a legitimate, FDA-approved medication with genuine therapeutic applications. Accessing it responsibly means doing more than locating a willing prescriber and a convenient pharmacy. It means ensuring that the clinical foundation for treatment is solid — that the organs responsible for processing the drug have been evaluated, that the parasitic target has been reasonably identified, and that no red flags have been missed in the rush to begin therapy.

Pre-treatment lab work is not the most visible part of the ivermectin access journey. But it may be the most important one. Patients who understand this — and who advocate for it — are not creating obstacles. They are exercising exactly the kind of informed engagement that leads to better outcomes.
